I confirm and agree that I wish to engage with N.I.K Nails and Spa Inc to apply for eyelash extensions. By checking this box, I understand and agree to following terms and conditions.

I understand that this procedure requires single synthetic eyelashes to be glued on my own natural lashes. *

I understand that some risks of this procedure may be but not limited to the eyes redness and irritation. The fumes from the adhesives may cause my eye tear up if i open my eyes *

I understand that it is my responsibility to keep my eyes closed and be still during the entire procedure until my technician addressed me to open my eyes *

I agreed to disclose my allergies that I may have Latex, surgical tapes, Cyanoacrylate, or vaseline *

I understand that I am required to follow the eyelash extensions care sheet in order to maintain the life of these extensions *

I agreed that by reading and signing this consent form, I release N.I.K Nails & Spa from any claims or damage of any natural included non-refundable

I agreed that I read and fully understand this consent form *

I am sound of mind and fully capable of executing this waiver of myself *

I give permission to N.I.K Nails & Spa to show my before and afters photos of eyelashes to potential clients *

I have read and completed the eyelash extensions agreed & consent form in this entirety, and have answered everything to the best of ability. I have been informed of potentially harmful or negative side effects that may be caused by the application or removal of eyelash extensions. *
